How can you get a proposal on your company's group insurance benefits?

It's as easy as 1, 2, 3!

1. click on Employee Data Sheet and print it.

2. Complete the information on the Employee Data Sheet and fax it along with, if applicable, a copy of your current group insurance plan (an employee booklet will probably be sufficient). Please also fax us any Extended Health Care and Dental claims experience (for the past 3 years) that you should have, along with a copy of your current group insurance invoice.

3. We will then determine the appropriate insurance company for your plan and prepare a proposal. If we have any questions, we will contact you.
